Elderly & Disabled/Adults Waiver
This waiver is offered on a first come first serve basis.
This waiver is intended to assist the elderly and disabled adults with daily living activities needed to maintain his or her health and safety.
- Individuals age 65 or older and 22 or older who are disabled according to Medicaid Standards and who meet admission requirements for a nursing facility and can be served safely in the community.
- Personal Care Attendant
- Household Supports and Personal Supervision
- Personal Emergency Response Systems
- Environment Modifications
- Case Management (other resources)
- Individuals must meet Medicaid financial requirements.
- Individuals must meet Nursing Facility level of care with medical and social information submitted to support the determination.
