Long Term-Personal Care Services

LT-PCS is an optional service under the Medicaid State Plan available to both waiver and non-waiver recipients.

The purpose of this option is to enable Medicaid recipients whose needs would otherwise require placement in a nursing facility to remain safely in their homes.  The mission is to supplement the family and/or community supports that are available to preserve the independence of recipients in the community.

Who is Eligible?

To qualify individuals must be ages 65 or older, or 21 or older who meet the disability criteria established by the Social Security Administration.  All persons must be capable of participating in their own care and self-directing the services provided by the caregiver.  They must also meet the level of care of a nursing facility.

 Services Provided

Personal care services are those services that provide assistance with the activities of daily living (ADL) and the instrumental activities of daily living (IADL).

    • Activities of Daily Living include:

-Eating

-Bathing

-Dressing

-Grooming

-Transferring

-Medication reminders

-Ambulation

-Toileting

    • Instrumental Activities of Daily Living include:

-Light housekeeping (only areas of consumer use)

-Food preparation and storage

-Grocery shopping

-Laundry

-Scheduling appointments

-Accompaniment to medical appointments

-Accessing Transportation

Requirements

    • Individual must meet age requirements
    • Individual must be currently Medicaid eligible
    • Individual must be capable of directing the activities of caregiver
    • Individual must meet the level of care criteria for admission to a long term nursing facility
    • Must have a gross monthly income of $552 or less
    • Countable resources cannot be worth more than $2000
