disabilities, whose disability manifested between the ages 22 to 55, to live
independently in their own home in the community of their choice.

- Individuals with significant disabilities whose disabilty manifested
between the ages of 22 to 55
Living in a nursing home or other institutional setting
Threathening to enter a nursing home or other institutional setting
Unserved and underserved population

and Mobility Training
- Furnishing
and moving into a new apartment
- Purchasing
Specialized Equipment and Supplies

Assistance
- Finding
and/or Subsidizing a Roommate
Requirements:
- Individual must meet on set of disability
requirement
- Individual must have a significant disability
- Individual must be living in a nursing facility or
other institution or a risk of being institutionalized
- Call the RIL's Information and Referral Specialist and give the RIL
Specialist your information
- You will be placed on a waiting list
- When a slot is available and you are deemed eligible, you will be
contacted to be accessed for eligibilty
- If you are deemed eligible, your services will begin
